Strategic plan 2023-2028
Our strategic plan for 2023 to 2028 A Community of Care for Today and Tomorrow outlines how, together, we will work towards our vision of exceptional hospice palliative care for everyone in our community. We’re focused on four strategic directions:
- Inspire excellence in the development of hospice palliative care across our community… and beyond.
- Nurture, celebrate, and support our people.
- Grow our services and supports to be inclusive of everyone who is in need.
- Strengthen and sustain our capacity so that we can continue to serve our community for years to come.
2023-2024 annual report
A lot has happened in the last year since we officially launched our 2023-2028 strategic plan, Community of Care for Today and Tomorrow. The theme of this year’s report sums it up: together, make a difference. Partnerships have been key to everything we’ve accomplished. As we pause and reflect on year, we’re feeling gratitude to be part such an extraordinary community care.

Policies and procedures
Accreditation
In the summer of 2022, our hospice received another three-year CARF (Commission on Accreditation of Rehabilitation Facilities) award. The accreditation is a testament to our incredible staff, volunteers, physician partners, board members, and our collective commitment to quality care. The CARF International accreditation is a visible symbol that assures our community that we are committed to continually enhancing the quality of our services and programs.
