Physician team

The Dorothy Ley Hospice Community Palliative Physician Team provides expert, 24/7 care to individuals and families, in the community and at our residence. Led by our medical director, Dr. Louie Mavrogiannis, the team ensures that every person we serve receives dignified, personalized support grounded in equity and compassion.

Dr. Justin Chopra

Dr. Justin Chopra is a palliative care physician with the Dorothy Ley Hospice Community Team, where he has worked since 2020. He is a member of both the College of Family Physicians of Canada and the Royal College of Physicians and Surgeons of Canada.

He earned his medical degree from McMaster University, followed by a family medicine residency at the University of Toronto and a palliative care fellowship at the University of Ottawa. He also holds an MBA from the Schulich School of Business.

Dr. Chopra is committed to a patient-centred approach in supporting individuals and families facing life-limiting illness. Outside of work, he enjoys time with his wife, two young children, and their family dog.

Dr. Chris Gilchrist

Dr. Chris Gilchrist is a palliative care physician with the Dorothy Ley Hospice Community Physicians and a part-time academic family physician with the Trillium-Summerville Family Medicine Teaching Unit. He is an Assistant Professor in the Department of Family and Community Medicine at the University of Toronto.

Dr. Gilchrist earned his medical degree from the University of Toronto in 2012, followed by a family medicine residency at Trillium Health Partners in 2014. He is currently pursuing a Master of Education at the University of Ottawa. In addition to his clinical work, he is actively involved in undergraduate medical education and serves as the Associate Director of Clinical Skills for the University of Toronto MD Program.

Dr. Aamir Haq joined the Dorothy Ley Hospice Community Physicians in 2013 and considers it the highlight of his medical career. He supports both adult and pediatric palliative patients in their homes and in hospice during what can be the most vulnerable time in a person’s life.

He is also a family physician at Lakefront Clinic and is proud to care for patients across the lifespan—from newborns to seniors. A passionate teacher and mentor, Dr. Haq enjoys working with medical students and residents and serves as a certified Peer Supporter with the Canadian Society of Palliative Care Physicians.

Dr. Haq earned his medical degree from the Northern Ontario School of Medicine in 2010 and completed his family medicine residency and palliative care fellowship at the University of Toronto. He holds a Certificate of Added Competence in Palliative Care from the College of Family Physicians of Canada.

Outside of work, he enjoys spending time with his three children, staying active through hockey, skiing, and hiking, and pursuing interests in travel, personal finance, and music—especially guitar and drums.

Dr. Hormoz Hamrahi

Dr. Hormoz Hamrahi has been practicing palliative care at Dorothy Ley Hospice for over 18 years. He is grateful to be part of a team that helps support patients and their families through some of life’s most challenging times. He is the father of three young boys and enjoys biking and running.

Dr. Keith Johnstone

Dr. Keith Johnstone has been a palliative care physician with the Dorothy Ley Hospice team since 2020. He considers it a great privilege to care for patients and their families alongside such a dedicated team. Outside of work, he enjoys spending time with his wife, going for walks with his dog, and running.

Dr. Louie Mavrogiannis

Medical Director (Ex-Officio)

Dr. Mavrogiannis has provided family medicine and palliative care throughout the GTA for the last 30 years. His focus on palliative care comes from a belief that continuity of care incorporates all aspects of a persons’ lifecycle from the beginning to the end. He completed his MD Ph.D. at The University of Toronto, and is an adjunct lecturer in Faculty of Medicine, Department of Physiology. He has been awarded two consecutive Excellence in teaching awards from the Department of Physiology. He is the Medical Director of The Dorothy Ley Hospice and the former lead physician for The Dorothy Ley Hospice Community Outreach Palliative Program.

He is a member of the hospice board of directors which focuses on community outreach and fund development. Dr. Mavrogiannis is a member of the Hospice Palliative Care Association, the Canadian Society of Palliative Care and the Ontario Medical Association section on Palliative Care. He is involved with the Local Health Integration Networks through his administrative work with service delivery providers, long-term care homes and family medicine and community health centres. He previously chaired the multidisciplinary planning committee for Mississauga-Halton Community Care Access Centre.

Dr. Julie (Ioulia) Napolskikh

Dr. Julie Napolskikh has been part of the Dorothy Ley Hospice Community Physician team since 2016. She considers it a privilege to support patients and families as they navigate advanced illness. Outside of work, she enjoys time with her young family and stays active through running, yoga, and outdoor activities.

Dr. Nadia Salvo earned her medical degree from the University of Toronto, where she also completed a residency in family medicine and an enhanced skills year in oncology and palliative care at Princess Margaret Hospital. She feels privileged to be able to care for individuals and their families during such a difficult, yet important, time in their lives.

Dr. Philippe Toupin “Dr. Phil”

Dr. Phil discovered his passion for palliative care as a medical student at the University of Ottawa, where he learned the value of slowing down and truly connecting with patients and their families. He finds deep fulfillment in providing comfort and support to those navigating life’s most difficult moments. Each encounter reminds him of the importance of resilience and human connection. He is grateful to be part of such a remarkable and dedicated team.

Dr. Rachel Wortzman

Dr. Rachel Wortzman is a dedicated palliative care physician and passionate educator. In addition to her clinical work and teaching, she contributes to global health initiatives. Outside of work, she enjoys cooking, cycling, and crocheting, and finds balance and inspiration in time spent with her husband and two young children.

Dr. Jessica Zive

Dr. Jessica Zive joined the Dorothy Ley Hospice team in 2017 and considers it a privilege to support individuals and their families during this chapter of life. She is committed to providing compassionate care that honours each person’s unique journey, whether at home or in hospice.
