RESIDENTIAL CARE

Residential Care is provided to individuals in the last weeks of life. Our twelve-bed residential hospice is a home-like environment
where we provide 24/7 nursing and medical care to individuals at end of life whose care needs exceed the capacity of the care providers to be managed at home.

Residential care aims to allow death to occur naturally rather than prolong life or hasten death. Care is provided through a transdisciplinary team approach consisting of palliative care physicians, registered nurses (RNs), registered practical nurses (RPNs), personal support workers (PSWs), specially trained volunteers and a nutrition consultant whose primary focus of care is on pain and symptom management and personal care.

Our temporary location at McCall Centre, 140 Sherway Dr., still provides a home-like atmosphere with comfortable en suite bedrooms and a family lounge area.  In addition, the individual, their family members and caregivers have access to wellness services, spiritual care and bereavement support services. Person-centered care plans are developed with each individual based on his/her needs, preferences, hopes and goals.

Residential care is offered to individuals who are 18 years or older and…

  • Have a prognosis of less than three months
  • Have a Palliative Performance Scale (PPS) of 40 or less
  • Recognize that resuscitation is not a service that we provide
  • Understand that no extensive diagnostics or treatments are offered other than those required for symptom and pain management and comfort measures
  • Live in or have family members who live in the Greater Toronto area

Exceptions to these criteria may be assessed on a case by case basis. Under government guidelines, the average length of stay is anticipated to be 21 days or less.

Anyone applying to the Residential Program is required to have their attending health care professional complete a Common Palliative Referral Form and fax the completed form to (416-626-7285). You will receive a fax confirmation.
