COMMUNITY PROGRAM

Hospice services are available to adults (18 years and older) living with a life-limiting illness and reside within the Etobicoke community.

Note: This referral form is for psychosocial community hospice care only. The referral form is not for Home and Community Care Support Services (previously known as the LHIN) and not for the DLH Community Physician Team.

For Physician use only. There are specific requirements to be eligible for the DLH Community Physician Team program and referrals must be sent from your doctor.

Hospice Care is about Quality of Life

Hospice palliative care is an approach to care that aims to reduce suffering and improve quality of life for people who are living with a life-limiting illness. It is the relief of suffering through the provision of pain and symptom management, as well as psychological, social, emotional, spiritual and practical support. 

Hospice palliative care places the individual receiving care and their family at the centre of decision making. We provide individualized care, guided by the unique needs and goals of each person. These goals are identified through initial and ongoing exploration of what is important to the individual and their family. Individual centered care affords respect, dignity and choice.

– Care Coordination and Counselling
– Palliative Physician Care
– Spiritual Care
– Wellness Day Program
– Wellness Therapies
– Caregiver Support
– Grief and Bereavement Services
