Referral Criteria

Any individual, regardless of age or medical condition, can access support from The Dorothy Ley Hospice once they have been diagnosed with a life-limiting illness and are in need of palliative support services.  Members of their families or others who anticipate or have suffered a loss can also access complementary supports where available.  Individuals may be referred by a physician, care provider or may request services directly on their own by contacting the Hospice directly. 

The Dorothy Ley Hospice serves individuals based on our assigned service area based on Ministry of Health and Long Term Care guidelines. 

Although a part of the Mississauga-Halton LHIN, our catchment area extends into the Central LHIN, Central West LHIN and the Toronto Central LHIN.  We therefore serve individuals throughout the GTA and individuals with urgent needs can receive services wherever there is space available.
