Partners
We work closely with health care professionals and service providers in the community. Our affiliations include community care access centres, hospitals, health centres, doctors, nursing and home care agencies, pharmacies, churches and other hospices. We also develop programs with other health care organizations to educate professionals about advances in hospice palliative care.
With a focus on the individual we facilitate an integrated approach to care. The goal is to provide seamless support that address the needs of the individual when, where and how they want it. Our partners include health care professionals and volunteers who have prior experience working with life-threatening illnesses: RNs, physicians, RPNs, case managers, physiotherapy and occupational therapists, social workers, pharmacists, nutritionists, directors of local hospitals and health centres, funeral home directors, and members of the clergy.
